Bioidentical Hormone Replacement Therapy (“BHRT”) refers to the treatment of symptoms and disorders associated with menopause using hormones that are molecularly identical to hormones the body naturally makes. The therapy is tailored to the individual using saliva test and other means of identifying specific hormone imbalances and having a compounding pharmacies tailor a specific replacement therapy. In recent years, BHRT has gained popularity through the media because it is identified with being “natural” and specific to the individuals and thus safer as opposed to using non-bioidentical hormones. However, the American College of Obstetricians and Gynecologist, the Endocrine Society, the North American Menopause Society, the United States Food and Drug Administration, and the Mayo Clinic have taken the position that there is no difference in the benefits or risks of using bioidentical instead of non-bioidentical hormones for replacement therapy. The concern lays in the use of biodentical hormones from compounding pharmacies because there are currently no regulations or FDA guidance regarding their safety, purity and potency.

How does Hormone Replace Therapy work?
Hormones can be administered in a variety of ways, including skin and vaginal creams, pills, topical gels, vaginal rings and tablets, and transdermal patches. Though all preparations are molecularly identical before their use, estrogens administered orally are modified by the liver before entering the blood, while those entering through the skin are not. Creams and gels applied to the skin also enter the blood directly and without modification, but it is not clear how much is actually absorbed. Pharmaceutical compounding is frequently used to modify the dose, form, and additives of the preparations, based on the instructions from a health care practitioner.
